Swindon’s learning disability team consists of a group of individuals all of whom work tirelessly to maximise life experience for those we support, whatever their job role. The team is recognised for its supportive nature and is one that has adapted well to this new way of working, not only for those we support in the community but also towards each other. We are committed to providing good quality supervision and continuing professional development opportunities.
Within the team, we work within a multi-disciplinary framework with individuals living within a variety of settings to maximise their independence and life opportunities. Alongside the completion of Care Act assessments and reviews, this team offers an opportunity to develop knowledge and skills in the areas of capacity, DoLs and court work.
The successful applicant will have experience of, and a passion to work with, individuals who have a learning disability. Demonstrating a commitment to providing a person centred and strengths based approach to their work to enable individuals to maximise their independence and life opportunities.
About the role
Link workers work alongside other learning disability team members and are responsible for holding a small caseload. Developing person centred support plans that focus on building individual strengths, to ensure the effective implementation of plans that promote independence. Working with individuals, in conjunction with their families and other support networks, to achieve their personal outcomes and to live as independently as possible within their local community. Utilising local community resources, assisted employment opportunities to maximise social integration, learning, and thinking creatively about how their day-to-day aspirations and goals may be realised.
What we are looking for
The successful applicant will have experience of and a passion to work with young adults who may have a learning disability, physical disability or mental health needs and are committed to providing a person centred and strengths based approach to their work.
Candidates must be fluent in the English language (as a requirement of Part 7 of the Immigration Act – for the effective performance of a customer-facing role).
