We are currently seeking an empathetic and motivated Community Navigator to join our dynamic and successful Health and Wellbeing Team.

You will be responsible for supporting and coaching individuals to navigate through health, community and voluntary sector services in Swindon to improve their well-being and quality of life. By empowering service users, you will promote independence in the management of their long term condition and circumstances.

This is an opportunity to have a positive impact on the general health and wellbeing of people in Swindon. You will be connected to a local GP surgery and will be working alongside a multi-disciplinary team. Working at the heart of the community, you will be making a real difference to people’s lives by helping them choose and adopt healthier lifestyles so they live well and can manage independently.

You should have knowledge of the available community resources across the health, social care, voluntary and community sector. You will need a good understanding of the boundaries of confidentiality and safeguarding and ideally you will have previous experience of working in health, social care, voluntary or community sector environment.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act – for the effective performance of a customer-facing role).
